Building Nurse Capacity for Healthy Ageing

Living well, not just longer. That's the heart of Healthy North Coast's Building Nurse Capacity for Healthy Ageing Program. Across both primary care and residential aged care, nurses play an extraordinary role in helping people manage their health, live independently, and age with confidence.

This region-wide initiative champions early intervention, chronic disease management, and anticipatory care – giving nurses the skills, tools, and support to work at the top of their scope. By strengthening the workforce through targeted education, practical HealthPathways, and sustainable models of care delivery, this program empowers nurses to deliver the highest level of care.

We aim to strengthen the capacity and capability of clinical nurses who deliver care, aged care, and palliative care to older people across our region. This initiative is a cornerstone of Healthy North Coast’s Aged Care and Healthy Ageing program, which is focused on empowering the workforce through education, skills development, and sustainable models of care. This activity was developed by Primary Care Innovation P/L and is funded by Healthy North Coast through the North Coast PHN program.

New HealthPathways

Healthy North Coast is pleased to announce new HealthPathways designed with input from nurses and GPs to support nurse-led care. As part of our commitment to building workforce capability across our region, these pathways address healthy ageing - including early intervention to prevent chronic conditions and improved systems for delivering palliative care.

These pathways are designed to support clinical nurses and practices with evidence-based criteria, clinical referral criteria, and tips to help minimise patient wait times and improve outcomes.

By embedding these pathways into every day practice, we're creating stronger, more confident models of care at the point of service.

In addition, the Primary Care Impact pages offer a central library of Quality Improvement (QI) Topics and resources that support ongoing improvement plans and processes. Quality Improvement and teams can draw on pre-populated QI activities, design their own, with guidance from the topic file page – designed to make improvement efforts accessible, enjoyable, engaging and achievable.

Together, these resources create a powerful platform for nurses to build capacity, enhance capability, and lead healthy aging strategies across our region.

Telehealth in Residential Aged Care

The National Telehealth Training program, developed by PHNs and endorsed by Royal Australian College of General Practitioners, Australian College of Nursing and Australian College of Rural and Remote Medicine, provides comprehensive modules for RACH staff to effectively utilize telehealth services. Access training modules for both aged care teams and visiting clinicians.
